Telehealth Consent

Effective Date: December 2025

Telehealth involves the use of electronic communications and technology to provide healthcare services remotely. Telehealth services provided by Obas Lab Diagnostics & Wellness comply with Florida Statute §456.47 and all applicable federal laws.

By scheduling or participating in telehealth services, you acknowledge and agree to the following:

1. Nature of Telehealth Services

  • Prescription management, when clinically appropriate and permitted by law

2. Provider Licensure

All healthcare providers delivering telehealth services are licensed or authorized to practice in the State of Florida, as required by Florida law. Telehealth services are provided only to patients located in jurisdictions where such services are legally permitted.

3. Benefits of Telehealth

Telehealth services may provide:

  • Convenient access to care
  • Reduced travel and wait times
  • Improved continuity of care

4. Risks and Limitations

You understand that:

  • Telehealth may not be appropriate for all medical conditions
  • Technical failures, delays, or interruptions may occur
  • Some conditions may require in-person evaluation or referral

5. Privacy and Security

6. Emergency Situations

Telehealth is not appropriate for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room immediately.

7. Consent to Treatment

  • Acknowledge that you may withdraw consent at any time

8. Prescription Policy (Florida-Specific)
